〔目的〕青森県内の病院における災害および感染症危機発生時の受援体制の現状を明らかにすること。
〔方法〕青森県内のすべての病院（94施設）の看護部門長94名を対象に，2023年2月～3月に受援体制に関する無記名自記式質問紙調査を行った。内容は属性5項目，受援体制21項目，その他自由記載2項目とした。回収は郵送による直接回収を行った。分析は単純集計およびクロス集計を行い，自由記載については同一内容を整理し，カテゴリー化した。青森県看護協会研究倫理審査委員会による承認を受けて実施した。
〔結果〕回収数は52部であり，そのうち49部を分析対象とした（有効回答率52.1％）。受援計画を策定している，もしくは準備中と回答したのは15施設（30.6％）であり，そのうち派遣された看護職を含む指揮命令系統が決まっているのは7施設，派遣される職員の業務が明確になっているのは5施設，身分保障が明確になっているのは2施設であった。受援体制を高めるために必要なこととして，【院内の理解・協力】【体制整備】【業務内容・手順の整備】【受け入れ側の教育・訓練】【情報共有・ネットワーク】【地域との連携】【マネジメント力強化】の7カテゴリーが抽出された。
〔結論〕青森県内では受援計画を策定していない施設が半数以上を占めており，災害に備え早急に策定していく必要がある。その際は，地域全体で連携し地域の医療機能を維持していく視点が必要であると考える。 |

[Objective] To clarify the state of response systems for disaster and infectious disease outbreak at hospitals in Aomori Prefecture.
[Methods] A cross-sectional survey using an anonymous, self-administered questionnaire was conducted on 94 nursing department heads in hospitals throughout Aomori Prefecture from February to March 2023 to assess the state of disaster support systems. The questionnaire comprised five demographic items, 21 items related to disaster support systems, and two open-ended questions. Completed questionnaires were collected via post. Data were analyzed using simple and cross-tabulation methods. Open-ended responses were categorized based on common themes. The study was approved by the Aomori Prefectural Nursing Association Research Ethics Committee.
[Results] A total of 52 questionnaires were distributed and 49 were analyzed (response rate: 52.1%). Among the responding institutions,15(30.6%) reported developing or preparing disaster relief plans. Of these, only seven had established a command system that included the nursing staff, five had clearly defined job duties for dispatched personnel, and two had clarified their employment security. Seven categories were extracted as elements necessary to strengthen the disaster relief system: [understanding and cooperation within a hospital], [system development], [work content and procedures], [education and training for staff], [information sharing and networking], [cooperation with the community], and [strengthening management capabilities].
[Conclusions] A significant majority of healthcare facilities in Aomori Prefecture have not established disaster relief plans. To mitigate the potential impact of future disasters, expediting the development of such plans is crucial. Moreover, a regionwide collaborative effort is indispensable for preserving the continuity of medical services in the event of a disaster. |
