| 内容記述 | 〔目的〕COVID-19感染症病棟で1年間勤務した看護師はどのような心境を抱き，どのように心理的に変化したのかを明らかにする。 〔方法〕COVID-19感染症病棟開設時より勤務する看護師5名を対象に，グループインタビューを行い，質的帰納的に分析した。 〔結果〕心境として90のコードと35のカテゴリーが抽出され，12通りの心理的変化が見いだされた。最も多く語られたのは不安であり，【メディア放送で不安が助長される】【未知なるCOVID-19ウイルスへの不安】は，【患者がまた増えるかもしれない不安】【変わらぬ感染への不安】として残り，【自身の感染が家族へ伝播させるかもしれない不安】は【1年以上にわたる感染症病棟勤務の負担】となっていた。一方，【近くで見守ることができない不安】【PPE着脱を適切に行えていないという不安】は【カメラ越しで患者を観察できる安心感】【物品が整っていることへの安心感】へと変化していた。また，【院内職員からの差別や偏見に傷つく】【他病棟スタッフからの心無い言葉】は【適切な感染看護を認めてもらえた喜び】へ，【陽性患者を受け入れられない本音】は【初めてCOVID-19患者が退院した喜び】へ，【感染症病棟の看護師としての使命感】は【感染症病棟勤務というプライド】へと変化した。 〔結論〕COVID-19感染症病棟で1年間勤務した看護師の12通りの心理的変化から，感染症病棟看護師への支援として，感染の不安の低減，やりがいを高める，メンタルヘルスへの支援が重要であるとの示唆を得た。 |

| 内容記述 | [Objective] This study aimed to explore the emotional state and psychological changes experienced by nurses who worked in a COVID-19 infectious disease ward for a year. [Methods] Group interviews were conducted with five nurses who had been working in the COVID-19 infectious disease ward since its establishment. Responses were analyzed using a qualitative inductive approach. [Results] 90 codes related to nurses’ emotional states were extracted and categorized into 35 categories, from which 12 patterns of psychological change were identified. Anxiety was the most frequently reported emotion. "Anxiety amplified by media coverage" and "anxiety about the unknown nature of COVID-19" persisted as "anxiety that the number of infected patients might increase" and "anxiety about ongoing infection". "Anxiety that infection might spread family members" developed into "the burden of working in a COVID-19 infectious diseases ward for more than a year". In contrast, "anxiety about not being able to closely observe patients" and "anxiety about improper donning and doffing of personal protective equipment" changed into "a sense of security from being able to observe patients via cameras" and "a sense of security due to available equipment." Furthermore, "emotional distress caused by discrimination and prejudice from colleagues" and "thoughtless comments from staff in other wards" changed into "the joy of having appropriate infection-control nursing recognized." [Conclusions] Based on the 12 identified patterns of psychological change among nurses who worked in a COVID-19 infectious disease ward for one year, this study suggests that support for nurses in infectious disease wards should focus on reducing anxiety about infection, enhancing job satisfaction, and providing mental health support. |
