American football is known as a contact sport with a high risk of injury due to its characteristic intense physical contact. Since injuries affect not only a player's ability to continue competing but also their long-term heal t h and quality of life, establishing effective prevention strategies in sports settings is a critical challenge regardless of the competition level. While top leagues in the United States and Japan have established safety management systems with well-equipped environments and specialized medical staff, Japanese regional collegiate leagues often operate under resource constraints, such as small roster sizes and a shortage of medical staff. Under these circumstances, directly applying prevention strategies based on data obtained from top levels is not easy, and there is a need to construct sustainable prevention systems tailored to the realities of the field. However, the current reality is that evidence focusing on regional universities regarding injury incidence, unique risk factors, and prevention strategies that are both feasible and effective within limited resources remains insufficient.
Therefore, the purpose of this study was to propose an effective injury prevention model and verify its utility in resource-limited regional leagues through a process of integrating quantitative data and qualitative factors and returning the findings to the field. This study adopted van Mechelen's sequence of prevention of sports injuries as the theoretical framework and employed an explanatory sequential design of mixed methods research to integrate quantitative and qualitative data. A distinct feature of this study is the attempt to implement the series of injury prevention processes in a sustainable manner while minimizing the burden on the field by integrating them into the league's existing annual schedule and regular events.
[Methods and Results]
This study is a cross-sectional and longitudinal study targeting the Tohoku Collegiate American Football League and consists of five studies. Ethical approval was obtained from the Institutional Review Board of Aomori University of Health and Welfare. (Approval Numbers: 24012, 24015, 24020).
[Study 1] Risk Transformation after the COVID-19 Pandemic Based on Long-term Data
Using injury reports from games in the Tohoku Collegiate American Football League (2018–2024), we aggregated data on injury incidence rates, injury sites, and diagnosis names, and statistically examined trends using the chi-square test. Injury reports were created by game doctors at each game and consisted of the date of injury, position, timing, body part, type, diagnosis, play type causing the injury, and injury situation.
A transient increase in chest contusions was observed immediately after the resumption of activities in 2020. Subsequently, the proportion of concussions increased in 2023–2024, revealing the possibility that this has led to a long-term transformation of risk.
[Study 2] Examination of the Impact of Safety Seminars Using Data Feedback on Injury Trends
Using the league's injury surveillance data, we conducted safety seminars centered on data feedback and compared injury occurrence before and after the intervention using the chi-square test.
After the intervention, injuries preventable through conditioning, such as muscle cramps (e.g., via hydration), decreased. On the other hand, no reduction effect was observed for injury risks involving interpersonal skills, such as concussions. These results suggested the need to go beyond quantitative data feedback and develop an approach more tailored to the actual situation by reflecting the underlying realities of the field.
[Study 3] Qualitative Examination of Field Perceptions and Current Practices
Returning to the second step of the 'sequence of prevent ion' of sports injuries, we conducted semi -structured interviews with players and staff to explore the perceptions and practices behind injury occurrences. The data were analyzed using text mining and qualitative content analysis.
It was confirmed that while safety awareness in the field showed high interest in the skills of the tackler delivering the hit, awareness regarding defensive skills and chest protection for the player receiving the hit tended to be relatively low. This qualitative finding supports the importance of a comprehensive prevention strategy covering both offensive and defensive aspects, including risk management for the player receiving the hit, in response to the increase in chest contusions and concussions identified in Study 1.
[Study 4] Investigation of Injury Occurrence in the Practice Environment
To complement the first step of the 'sequence of prevent ion' of sports injuries, we conducted a survey on injuries during practice using a questionnaire and analyzed the trends using descriptive statistics.
Unlike the game data, t rends in the practice environment showed a high occurrence of injuries among first- and second-year students, as well as injuries to fingers and thighs. This suggests that safety management more aligned with the reality of the field becomes possible by evaluating risks specific to the practice environment in addition to analyzing game injury data.
[Study 5] Returning Integrated Findings and Verifying Consciousness Change
We integrated the quantitative data, qualitative perceptions, and practice environment risks obtained in Studies 1–4 and implemented an educational intervention by feeding back this information through the league's regular seminars.
The results suggested that the participants' sense of ownership may have been fostered, as improvement s in knowledge regarding environmental and systemic risk management and an increased intent ion to introduce safety measures were confirmed. This is considered the result of providing information that combines objective data with narratives from the field, which prompted new realizations and a re-recognition of the importance of risks that tend to receive relatively less attention.
[Conclusion]
This study clarified the utility of a cyclical injury prevent ion model that integrates quantitative data and qualitative perceptions in the Tohoku Collegiate American Football League, where support resources such as medical staff are limited. The achievement of this study lies in highlighting aspects of safety awareness in the field that tend to receive relatively less attention by integrating quantitative data on injury conditions and qualitative data from interviews, and reflecting these findings in feedback to the field in a manner tailored to the current situation of regional leagues. The process of returning these integrated findings to the field by incorporating them into the league's existing schedule was effective in evoking a sense of ownership among participants and increasing their motivation for behavioral change in safety management, while minimizing new operational burdens.
In conclusion, the injury prevention model of this study, which integrates both quantitative and qualitative approaches and circulates them according to the reality of the field, provides empirical evidence for promoting learning and motivation for behavioral change in safety management in resource-limited organizations. This model has generalizable applicability for fostering a safety culture not only in American football but also in other contact sports and community sports organizations that involve physical contact yet face resource constraints. It is hoped that this study will serve as an aid in discussing more effective safety measures tailored to the current status of regional leagues.
