| 内容記述 | [Introduction] The human foot consists of three arch structures and plays an important role in shock absorption and propulsion during gait and weight bearing. While evaluation methods for the medial longitudinal arch (MLA) and its relationship with locomotor activities have been widely reported, quantitative assessment methods, functional roles, and clinical significance of the lateral longitudinal arch (LLA) remain poorly understood. Quantitative evaluation of the LLA may improve understanding of its biomechanical function and support clinical applications such as insole prescription and evaluation of therapeutic exercise effects. Recently, ultrasound imaging (US), which enables visualization of deep structures without radiation exposure, has been increasingly used in musculoskeletal assessment. Therefore, US may be a useful tool for LLA evaluation. The purpose of this study was to develop a quantitative US-based assessment method for the LLA and to clarify factors influencing this assessment, as well as LLA behavior under different postural, loading, and foot morphology conditions. [Overview of Each Study] Study 1: Development of an Ultrasound-Based Assessment Method for the LLA [Methods] This cross-sectional study included 21 university students (14 males, 7 females). Cuboid Height (CH) was measured using radiography and ultrasound imaging. A custom measurement platform with two 20-cm-high platforms separated by a 6-cm gap was constructed based on previous studies. Participants stood with equal weight on both feet while the ultrasound probe was inserted through the gap to measure CH. Validity was assessed using correlation and Bland–Altman analyses, and intra- and inter-rater reliability were evaluated using intraclass correlation coefficients (ICCs). [Results] A strong correlation was found between radiographic and ultrasound measurements of CH (r = 0.98, p < 0.01). Bland–Altman analysis showed a fixed bias of −0.71 mm (95% CI: −0.96 to −0.46 mm). Intra- and inter-rater reliability were excellent (ICCs = 0.98 and 0.99). [Conclusion] Ultrasound-based CH measurement is a noninvasive and highly accurate method for assessing the LLA and a viable alternative to radiography. Study 2: Factors Associated with Cuboid Height [Methods] Seventy-three university students (43 males, 30 females) participated. Age, height, body mass, foot length, CH, Dorsal Arch Height (DAH), and the Foot Posture Index-6 (FPI-6) were measured. Sex-specific correlations were calculated, followed by multiple regression analysis using combined data. [Results] CH was significantly greater in males. In males, CH correlated with height, body mass, DAH, and FPI-6, while in females it correlated with DAH and FPI-6. Multiple regression analysis identified DAH and sex as significant predictors of CH (DAH: β = 0.911; sex: β = 0.290; adjusted R² = 0.531; p < 0.001). [Conclusion] CH is influenced by DAH and sex, indicating sex-specific reference values. Although functionally related to the midfoot, CH reflects LLA characteristics not captured by the FPI-6. Study 3: Sex Differences in Changes in Cuboid Height under Different Loading and Postural Conditions [Methods] Forty-seven university students (26 males, 21 females) participated. CH and DAH were measured under four conditions: sitting, standing, one-leg standing (OLS), and one-leg squat at 90° knee flexion (OLSQ90°). A split-plot two-way analysis of variance was conducted with sex and posture/loading condition as independent variables, followed by post hoc multiple comparisons. [Results] A significant main effect of condition was observed for CH. Post hoc analysis showed that CH decreased progressively with increasing load and postural demand (Sitting: 19.79 ± 3.25 mm; Standing: 18.74 ± 3.32 mm; OLS: 18.39 ± 3.49 mm; OLSQ90°: 18.09 ± 3.34 mm; p < 0.001). [Conclusion] Both CH and DAH significantly decreased with increasing load; however, the magnitude of change was small, suggesting that the foot arch adapts to loading through a truss-like mechanism. Study 4: Differences in Cuboid Height among Foot Posture Types under Different Loading and Postural Conditions [Methods] Forty-seven university students were classified into pronated, normal, and supinated foot groups using the FPI-6. Measurement variables and experimental procedures were identical to those used in Study 3. A split-plot two-way analysis of variance was performed with sex and posture/loading condition as independent variables, followed by post hoc multiple comparisons. [Results] A significant main effect of foot posture was observed. The supinated foot group demonstrated significantly greater CH than the neutral and pronated foot groups (Supinated: 21.82 ± 3.26 mm; Normal: 18.28 ± 3.15 mm; Pronated: 17.40 ± 2.62 mm; p < 0.010). [Conclusion] Supinated feet exhibited a significantly higher CH, indicating a higher LLA structure. No significant difference in CH was observed between pronated and neutral feet, suggesting that a reduction in MLA height does not necessarily accompany a reduction in LLA height. |
