{"created":"2023-06-19T09:22:09.372338+00:00","id":765,"links":{},"metadata":{"_buckets":{"deposit":"4b272b29-a7fd-4587-a57b-de8c55af7f70"},"_deposit":{"created_by":4,"id":"765","owners":[4],"pid":{"revision_id":0,"type":"depid","value":"765"},"status":"published"},"_oai":{"id":"oai:auhw.repo.nii.ac.jp:00000765","sets":["86:115"]},"author_link":["4168","4169"],"item_10002_alternative_title_1":{"attribute_name":"その他(別言語等)のタイトル","attribute_value_mlt":[{"subitem_alternative_title":"Empathlc Process in Patient-Nurse Relationship and Its Affectors"}]},"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2001-03","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"132","bibliographicPageStart":"119","bibliographicVolumeNumber":"2","bibliographic_titles":[{"bibliographic_title":"青森県立保健大学紀要"},{"bibliographic_title":"Journal of Aomori University of Health and Welfare","bibliographic_titleLang":"en"}]}]},"item_10002_description_19":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"application/pdf","subitem_description_type":"Other"}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"現在、看護学分野において、ケアリング、癒しといった概念が重要視され、患者-看護婦関係、中でも共感の重要性が改めて注目されている。しかし、看護婦は実践場面でこのことに配慮して関わっていながら、その概念の曖昧さから効果的に活用できない現状にあり、その明確化が求められている。本研究では、東北地方の某県立病院内科系病棟に勤務する看護婦31名を対象に、半構成的質問紙による面接によって、共感プロセスとその影響因子を帰納的に導き出した。また、共感プロセスの各段階と情動的共感性尺度との関連性を検討した。結果、共感プロセスは『第一印象』、『独自性の気づき』、『共感的理解』、『積極的関わり』:苦痛を和らげたい思い→関わり→患者の変化、『信頼関係の深まり・成長』と連むことが明らかになった。共感プロセスには、患者側の要因である「個室」「重症度」「患者・家族の受け止め方」、看護婦側の要因である「経験」「姿勢」「看護ケア体制」が影響していた。また、この共感プロセスの概念枠組みに洽って、本研究対象者の語った患者関係について段階別に3群に分類した情動的共感性尺度得点には有意差がなく、看護における共感スケール開発の必要性が示唆された。以上から、本研究ではふだんの実践で看護婦があまり意識していない共感プロセス、影響因子を明確にでき、トラベルビーや小代のモデルを犬枠において検証し、積極的関わりの部分をより明細化できたと考える。\nConcepts such as caring and healing are currently being emphasized in the field of nursing and the importance of the patient-nurse relationship, and especially empathy, has attracted attention again. However, although nurses take care of patients by empathizing in actual practice, empathy cannot be effectively utilized due to the ambiguity of the concept under present circumstances. This concept must be clearly defined. In this study, the empathic process and its affectors were determined inductively by interviewing 31 nurses who worked in the medical ward of a certain prefectural hospital in the Tohoku region of Japan using a semi-structured questionnaire. In addition, the relationship between the respective steps of the empathic process and an Emotional Empathy Scale were examined. Results revealed that the emphatic process proceeded as follows: \"first impression\", \"independent awareness\", \"empathic understanding\", \"positive commitment\" (desire to relieve patient's pain → commitment → changes in patient), and \"deepening and growth of relationship with mutual trust\". \"Private room\", \"severity\", and \"an attitude of reception by the patient and family\" were factors on the patient's behalf while \"experience\", \"attitude\", and the \"nursing care system\" on the nurse's behalf that affected the empathic process. There was no significant difference in the score on the Emotional Empathy Scale, which classified the patient-nurse relationship described by the subjects of this study into 3 groups along the conceptional framework of the empathic process, suggesting the necessity of development of an empathy scale in nursing.
