| 内容記述 | 看護師がイノベーションと認識し、かつ、研究的根拠が明白な看護技術の採用程度とその看護師が所属する看護部の組織的要因との関連性について探求することを目的に、病床数規模別に無作為抽出した看護部責任者141名、及び、各病院10名のスタッフナース計1410名を対象に調査した。スタッフには根拠のあるイノベーティブ看護技術22項目について、E.M.Rogersの普及決定過程における段階モデルを用い質問し採用度を算出した。責任者には、個人的特性の他、雇用状況、研修費用、病院の管理運営会議への参加、研修機会、情報収集の手段、地域交流、専門看護師の勤務形態等、24項目の組織特性を質問し、返送された127部(90.1%)中、有効回答124部、スタッフナースの有効回答886部を分析対象とした。その結果、イノベーティブ看護技術の採用度と看護部の組織特性の関連では、「専門看護師や認定看護師の配置」*、「病院機能評価をうけている」*、「教育・研究機関の併設」**、「院内教育プログラムやセミナーの公開」**、「院内情報ネットワークの整備」*、「文献検索手段としてのインターネットの利用」*、「院外研修の伝達共有の場」**の質問項目で、有意な関連が認められた。さらに、「看護部の意志決定を委員会に委譲することがある」*、「専門性の高い看護師の勤務形態の工夫」***という回答は、組織特性であると同時に、その組織特性を形成する責任者個人の特性としても解釈でき、改めて、看護部責任者の姿勢の重要性が明らかになった。*:p<0.05、**:p<0.01、***:p<0.001 The purpose of this study is to analyze the relationship between the degree of innovative nursing practices and the organizational factors which are currently being employed by the nurses in the hospitals. These innovative practices are recognized by the nurse as innovative skills that are supported by scientific research. The subjects were 141 randomly selected nurse administrators from various hospital sizes and 10 staff nurses from each of those selected hospitals, or 1410 staff nurses. The Diffusion of Innovation Model by E.M. Rogers was used to determine the diffusion of each 22 innovative practices in a variety of hospitals. In order to identify organizational characteristics, 24 questions were given to the nurse administrators regarding individual personalities, position vacancies, budget for nurses' education, attendance to administrative meetings, educational opportunities, methods to obtain information, communication with communities, working shift for CNSs (Certified Nurse Specialists), and others. There were 127 (90.1%) total responses from nurse administrators in which there were 124 valid responses that were analyzed with 886 responses from staff nurses. There were strong correlations between innovative practices and organizational factors such as, "employment of CNSs or Certified Nurses," "updated hospital accreditation," "alliance with education or research facilities," "in-service educations and program/seminars that are open to the public," "technologically equipped information network," "opportunities to share educational information outside of the hospital." Although "delegation to committees for decisions in the nursing department" and "flexible working schedules for nurse specialists" are categorized into organizational factors, they also explain the individual characteristics of the nurse administrators and its impact on innovative practices. |
