| 内容記述 | 糖尿病患者は、糖尿病の療養のため、今までの生活習慣を変え療養行動を生活に組み入れることが求められる (行動変容)。しかし行動変容の必要性を認識していても行動を実際に変えることは難しい。そこには、自己存在価値と行動変容ができない原因の帰属が関連していると考えた。そこで糖尿病患者7事例の療養行動の考え方と自己存在価値について学習理論の視点から分析した。その結果以下のことが示唆された。1) 自己存在価値の捉え方は、「他者との比較による価値付け」と「自身による価値付け」という2つに特徴づけられた。(1)「他者との比較による価値付け」をする人は、自己存在価値観が低く、行動変容できない原因が外的統制要因に帰属していた。(2)「自身による価値付け」をする人は、自己存在価値観が高く、行動変容できない原因が内的統制要因に帰属していた。2) 療養行動の目標では、「他者との比較による価値付け」をする人は「今を無理しない・楽に」、「自身による価値付け」をする人は「これからを考える」という特徴があった。3) 療養行動の評価の視点は、「他者との比較による価値付け」をする人は結果で判断する、「自身による価値付け」をする人はプロセスで判断するという特徴があった。看護職者が糖尿病患者の行動変容を支援する場合、行動変容ができない原因の帰属傾向、自己存在の価値付け、及び療養行動の目標と評価の視点を理解し、その情報を患者教育に活用することで糖尿病患者はエンパワメントされることが示唆された。 In order to control their illness, diabetics need to modify their lifestyles and incorporate therapeutic behaviors into their lives (behavioral modification). However, even when diabetics recognize the necessity for behavioral modification, it is often difficult for them to actually change their actions. Self worth is thought to relate to the causal attribution of failed behavioral modification. Consequently, the relationship between therapeutic behaviors and self-worth was analyzed in seven diabetics from the viewpoint of learning theory. The results suggested the following : 1) Self-worth was divided into that "based on others' viewpoint" and that "based on one's own viewpoint". Diabetics whose self-worth was based on others' viewpoint demonstrated low self-worth, and the cause of failed behavioral modification was attributable to external control factors. Diabetics whose self-worth was based on their own viewpoint exhibited high self-worth, and the cause of failed behavioral modification was attributable to internal control factors. 2) Regarding the objectives of therapeutic behaviors, "based on others' viewpoint" group tended to think about avoiding strain or making life easier, while "based on one's own viewpoint" group tended to think about the future. 3) Furthermore, in terms of assessment of therapeutic behaviors, "based on others' viewpoint" group tended to judge based on outcome, while "based on one' s own viewpoint" group tended to judge based on process. Therefore, when nurses support the behavioral modification of diabetics, they can empower patients by ascertaining their view of self-worth, discerning the objective and assessment of therapeutic behaviors, and then effectively utilizing the resulting information for patient education. |
