| 内容記述 | 本研究の目的は、精神科看護師が保護室入室患者に対して行っているクリニカルジャッジメントについて探求することである。今回は、経験5年以上の看護師5〜6名を対象に、2つの病院でフォーカスグループインタビューを実施し、保護室に入室中の患者から出される要求への判断と対応方法を抽出した。その結果、観察の視点、開放要求の受け入れに関与する背景、要求へのかかわり方、開放を要求する患者へ抱く感情、看護師の役割範囲の5カテゴリーが構成された。看護師は、患者の要求に対応する際に、対人関係や生活リズム等の情報や入室した理由となる問題状況の改善について、経時的に情報を収集していた。さらには、感情的な葛藤、保護室が設置されている病棟の条件という物理的要因、看護師の責任範囲という限界が判断に影響することも示唆された。 The purpose of this research is to explore the clinical judgments which psychiatric nurses make on patients in seclusion rooms. The focus group interviews were carried out in two hospitals. The participants were 5-6 nurses in each hospital who have psychiatric nursing experience of 5 years or more. The narrative texts were interpreted by the researcher and the judgments and the responses to the requests from patients in seclusion rooms were extracted. As a result, the following 5 categories emerged : 1) The observation point ; 2) The backgrounds which influence the judgment of acceptance of a patient's request to get released ; 3) The way of being concerned with the requests ; 4) The nurse's feelings to the patient, who demands to get released ; 5) The range of the role of the nurse. Nurses received multifarious information about the verbal and/or nonverbal cues of the patient and the self care patterns when they responded to the requests of the patient. Also, nurses obtained information in sequence about the improvement of the status of the patient's condition which was the reason they were secluded. Moreover, it was suggested that the feelings, physical factors, and the limit in the responsibility range of the role of the nurse, and the emotional conflict influenced the judgments that they rendered. |
