| 内容記述 | 本研究の目的は、出産を経験した40〜50代の女性の分娩体験からケアニーズを明らかにし、分娩期におけるドゥーラの役割を探ることである。なおドゥーラとは、産前・出産中・産後の母親を身体的にも、情緒的にも、継続して支援し情報を提供する経験豊かな女性(家族や親族以外)をいう。分娩経験者で40〜50代の女性12名を対象にフォーカスグループインタビューを実施した。面接では、自分自身の出産について、その時の思い、出産時に辛かった事や他者から受けて心地よかった事等についてデータ収集した。分析は内容分析の手法をもとに、得られたデータから分娩期のケアニーズを抽出した。その結果、分娩期の女性のケアニーズには(1)適切な時期に安心して分娩入院できる、(2)分娩の進行状況が理解できる、(3)分娩進行中誰かに付き添ってもらえる、(4)分娩進行に合わせて主体的行動が取れる、(5)医療者との信頼関係を形成する、(6)辛さや不安を共有してもらえる、(7)家族から心理的な支援が得られる、(8)必要な医療処置を納得して受けられる、(9)分娩体験を肯定的に受けとめられるがあった。またこれに対するドゥーラの役割には、(1)分娩中の女性が独りにならないよう側にいて支持する、(2)分娩中の女性と信頼関係を形成し、代弁者となる、(3)状況や今後の見通しを理解できるよう支援する、(4)苦痛を共感し緩和を援助する、(5)分娩中の女性と専門家との関係を支援する、(6)分娩中の女性と夫(家族)との関係を支援する、(7)女性の分娩体験を支持することがあると考えられた。 The goal of this study was to clarify care needs based on the childbirth experiences of women in their 40's and 50's, and to investigate the role of doula throughout labor. A doula is defined as an experienced woman (other than a family member or a relative) who provides continuous physical and emotional support and information to mothers before, during and after childbirth. Group focus interviews with twelve women in their 40's and 50's who had experienced childbirth were carried out and subjected to content analysis. As a result, the care needs of women in labor include (1) hospitalization for labor at the appropriate time without anxiety, (2) understanding the progress of labor, (3) having somebody to be present during labor, (4) taking independent action in line with the progress of labor, (5) forming a trusting relationship with medical personnel, (6) sharing the pain and unease, (7) obtaining psychological support from family, (8) receiving necessary medical treatment with their consent, and (9) childbirth experience positively appreciated. In addition, in this regard, the role of doula is thought to include, (1) staying with the woman so that she is not alone, and supporting her, (2) forming a trusting relationship with the woman in labor, and acting as her voice, (3) helping to understand the situation and outlook, (4) empathizing with, and helping to alleviate the pain, (5) supporting the relationship between the woman in labor and the experts, (6) supporting the relationship between the woman in labor and her husband (family), and (7) assist the labor experience of the woman. |
